Figure out the reply to "PlanExplainsStop" once when we stop and then use the cached value. This fixes problems, for instance, with the StepRange plans, where they know that they explained the stop because they were at their "run to here" breakpoint, then deleted that breakpoint, so when they got asked again, doh! I had done this for a couple of plans in an ad hoc fashion, this just formalizes it. Also add a "ResumeRequested" in Process so that the code in the completion handlers can tell the ShouldStop logic they want to resume rather than just directly resuming. That allows us to handle resuming in a more controlled fashion. Change the AppleObjCTrampolineHandler to always run all threads when resolving the target of an ObjC method call. We sometimes need to be able to call functions (via Process::RunThreadPlan) from code run on the private state thread. To do that we have to spin up a temporary "private state thread" that will respond to events from the lower level process plugins. This check-in should work to do that, but it is still buggy. However, if you don't call functions on the private state thread, these changes make no difference. This patch also moves the code in the AppleObjCRuntime step-through-trampoline handler that might call functions (in the case where the debug server doesn't support the memory allocate/deallocate packet) out to a safe place to do that call. git-svn-id: https://llvm.org/svn/llvm-project/lldb/trunk@154230 91177308-0d34-0410-b5e6-96231b3b80d8

Convert lldb::ModuleSP to use an instrusive ref counted pointer. We had some cases where getting the shared pointer for a module from the global module list was causing a performance issue when debugging with DWARF in .o files. Changing the ObjC find method implementation to use a ClangUtilityFunction inserted into the target. Consolidate all the logic for finding the target of a method dispatch into this function, insert & call it. Gets calls to super, and all the fixup & fixedup variants working properly.
