gallivm: Updated lp_build_log2_approx to use a more accurate polynomial. Tested with lp_test_arit with 100% passes and piglit tests with 100% pass for log but some tests still fail for pow. Signed-off-by: José Fonseca <jfonseca@vmware.com>

gallivm: Import the code to compute the minimax polynomials. It's quite a pain to remember the details after a while, and it is quite likely we'll want to use this again, either for different polynomial orders or different functions, so commit it here.
