Separate creation time & flush time behavior in GrDrawOpAtlas (take 3)
This CL clarifies what is going on in the GrDrawOpAtlas and GrAtlasGlyphCache.
For the GrDrawOpAtlas:
At creation time all the allowed pages are created (with their backing GrTextureProxies) but they aren't instantiated.
The GrDrawOpAtlas::instantiate call is called in preFlushCB and allocates any pages known to be needed at the start of flush
GrDrawOpAtlas::addToAtlas is called at flush time and, if a new page is activated, will instantiated it at that time.
During compaction, an unused page will be deInstantiated but its Plots and backing GrTextureProxy will remain alive.
The GrAtlasGlyphCache reflects the changes to the GrDrawOpAtlas
It now carries a GrProxyProvider for when it needs to create an atlas
It passes in a GrResourceProvider* at flush time to allow instantiation.
It does not, yet, allocate that GrDrawOpAtlases it might ever require.

Remove tracking of SkDestinationSurfaceColorMode on GrTextureProxy for mips
When we create a new surface from a proxy we were always using legacy and
when we wrapped an existing surface we never actaully had to use the value
we pulled off of it since the proxy was already instantiatied.

Make non-ddl lazy proxys clean-up and delete their callbacks immediately after instanstation.
This makes sure resources are released and free'd as soon as possible if we
no longer need them.

Have lazy proxies keep their callbacks around and clean up their lambdas in the dtor
I believe after this CL we will be at a place where we just have to null out the
fTarget of a lazy proxy and it will reinstantiate itself.

Fix drawSpecialImage
W/o this fix the strict constraint on the specialImage draws can sometimes be removed. This CL ensures that the temporary SkImage always has the actual dimensions of the special Image.
Note: the replacement of full screen clears w/ draws revealed this bug b.c. the image filters were only drawing a rect for the content area of the special images. Thus when the final DAG result was drawn to the canvas some of the bleed through (from the removal of the strict constraint) was showing.

Add option to create a deferred render target context with mips
We need this since we have texture generators that draw the base level
but nothing more. Thus we want them to be able to directly draw into
a pre allocated mipped target instead of doing a copy later.

Move GrResourceProvider::createMipMappedTexture to GrSurfaceProxy::MakeDeferredMipMap
Although this adds a new method to GrResourceProvider, it is a slight improvement in that it removes some GrProxy stuff from GrResourceProvider (which arguably should only deal in GrSurface-derived classes).

Fix MDB regression

Make instantiate return a Boolean

Partially defer SkImage_Gpu
One of SkImageCacherator, GrBitmapTextureMaker, GrImageTextureMaker, GrTextureAdjuster, GrTextureProducer or SkImage has to take the first step. This is probably the least odd of the options.

Rebase and fix chromium

Disable deferred gpu resources for Chrome branch
Chrome's branch is coming up and it seems unlikely that I will be able to push instantiation all the way to flush time (where failure will be easier to deal with) before then. This CL should silence the P1 bug but could introduce layout test diffs.
Ideally this wouldn't land until after: https://codereview.chromium.org/2718353004/ (Add flag for upcoming Skia change) so the layout test differences will be visible on the DEPS roll

Fix gpu blurring on platforms that "useDrawInsteadOfClear"
Platforms that "useDrawInsteadOfClear" take a different path in
GrRenderTargetContext::internalClear. The different path involves
a lot of comparisons of the drawn rect with the bounds of the RenderTarget. Since the RenderTargets are now deferred (and the instantiated version might be larger than the proxied size) case must be taken in prematurely optimizing away clears and draws.

Added GrSurfaceContext and GrTextureContext
This lets copy-to-texture to be treated like copy-to-rt.
To match current behavior, though, copies to texture are
still executed immediately (forcing a flush).
Once MDB is enabled, copies to texture will be deferred.

Add explicit UniqueID classes for GrGpuResource & GrSurfaceProxy
This sets the stage for using the Proxy's/RenderTargetContext's ID above the flush and the RenderTarget's/GrGpuResource's below the flush.

Consolidate GrSurfaceProxy Make methods
I think GrSurfaceDesc is still the most compact way to communicate the deferred GrSurface's settings to the Proxy but this CL, at least, reduces where it is used.

* Skips non-AA diff rect elements and replaces them with window
rectangles.
* Places window rectangles in the interiors of antialiased diff rects.

Have GrSurfaceProxys and GrGpuResources draw from the same pool of unique ids
The idea here is that, for wrapped Proxy objects, we want the uniqueID to reflect that of the wrapped object. For this to work the IDs for the non-wrapped versions can't conflict with GrGpuResource's pool of IDs.
