video: omap: convert drivers/video/omap/* to use module_platform_driver()
This patch converts the drivers in drivers/video/omap/* to use the
module_platform_driver() macro which makes the code smaller and a bit
simpler.

OMAP: OMAPFB: split omapfb.h
Split arch/arm/plat-omap/include/mach/omapfb.h into two files:
include/linux/omapfb.h - ioctls etc for userspace and some kernel
stuff for board files
drivers/video/omap/omapfb.h - for omapfb internal use
This cleans up omapfb.h and also makes it easier for the upcoming new
DSS driver to co-exist with the old driver.

omap: headers: Move remaining headers from include/mach to include/plat
Move the remaining headers under plat-omap/include/mach
to plat-omap/include/plat. Also search and replace the
files using these headers to include using the right path.
This was done with:
