[S390] qdio: Split SBAL entry flags
The qdio SBAL entry flag is made-up of four different values that are
independent of one another. Some of the bits are reserved by the
hardware and should not be changed by qdio. Currently all four values
are overwritten since the SBAL entry flag is defined as an u32.
Split the SBAL entry flag into four u8's as defined by the hardware
and don't touch the reserved bits.

[SCSI] zfcp: Merge FCP task management setup with regular FCP command setup
For task management commands, only LUN and flags are required. The
regular FCP setup already sets the LUN in the fcp_cmnd. All is
required for merging the function is setting up the TM flags.

[SCSI] zfcp: Replace kmem_cache for "status read" data
zfcp requires a mempool for the status read data blocks to resubmit
the "status read" requests at any time. Each status read data block
has the size of a page (4096 bytes) and needs to be placed in one
page.
Instead of having a kmem_cache for allocating page sized chunks, use
mempool_create_page_pool to create a mempool returning pages and
remove the zfcp kmem_cache.

Interrupting the connection to the FCP channel while I/O requests are
being issued can lead to this deadlock. scsi_dispatch_cmd already

description in "[PATCH] Eliminate error handler overload of the SCSI
serial number", the serial_number is not used, so simply drop the

[SCSI] zfcp: Correct false abort data assignment.
The request data assignment between the fsf abort initiator and its
corresponding handler is not consistent and leads to an unpredictable
behaviour, e.g. kernel panic. This patch fixes this issue and assigns
the correct value.

[SCSI] zfcp: Fix adapter activation on link down
If an exchange config is executed while the local link is down, the
request succeeds but the returned data is incomplete. Proceeding with
the adapter activation is leading to an unpredictable behaviour (e.g.
kernel panic) caused by invalid values. In such a scenario the
recommended ERP is to retry the action and wait for a link up event.
If the issue persists the activation has to fail.

[SCSI] zfcp: Use SCSI device data zfcp_scsi_dev instead of zfcp_unit
This is the large change to switch from using the data in
zfcp_unit to zfcp_scsi_dev. Keeping everything working requires doing
the switch in one piece. To ensure that no code keeps using the data
in zfcp_unit, this patch also removes the data from zfcp_unit that is
now being replaced with zfcp_scsi_dev.
For zfcp, the scsi_device together with zfcp_scsi_dev exist from the
call of slave_alloc to the call of slave_destroy. The data in
zfcp_scsi_dev is initialized in zfcp_scsi_slave_alloc and the LUN is
opened; the final shutdown for the LUN is run from slave_destroy.
Where the scsi_device or zfcp_scsi_dev is needed, the pointer to the
scsi_device is passed as function argument and inside the function
converted to the pointer to zfcp_scsi_dev; this avoids back and forth
conversion betweeen scsi_device and zfcp_scsi_dev.
While changing the function arguments from zfcp_unit to scsi_device,
the functions names are renamed form "unit" to "lun". This is to have
a seperation between zfcp_scsi_dev/LUN and the zfcp_unit; only code
referring to the remaining configuration information in zfcp_unit
struct uses "unit".

[SCSI] zfcp: Trigger logging in the FCP channel on qdio error conditions
Exploit the cio siosl function to trigger logging in the FCP channel
on qdio error conditions. Add a helper function in zfcp_qdio to ensure
that tracing is only triggered once before calling qdio_shutdown.
Trigger in zfcp for hardware logs are:
- timeout for FSF requests to the FCP channel
- "no recommendation" status from FCP channel
- invalid FSF protocol status
- stalled outbound queue
- unknown request id on inbound queue
- QDIO_ERROR_SLSB_STATE
All of the above triggers run from the Linux qdio softirq context, so
no additional synchronization is necessary for the handling of the
ZFCP_STATUS_ADAPTER_SIOSL_ISSUED flag.

[SCSI] zfcp: Introduce experimental support for DIF/DIX
Introduce support for DIF/DIX in zfcp: Report the capabilities for the
Scsi_host, map the protection data when issuing I/O requests and
handle the new error codes. Also add the fsf data_direction field to
the hba trace, it is useful information for debugging in that area.
This is an EXPERIMENTAL feature for now.

[SCSI] zfcp: Cleanup function parameters for sbal value.
A lot of functions require the amount of SBALs as one of their
parameter which is most times invariable. Therefore remove this
parameter and set the SBAL value explicitly if a non standard value is
required. In addition the warning message "oversized data" is
replaced with a BUG_ON() statement assuring the limits defined and
requested by zfcp.

Commit 64deb6efdc5504ce97b5c1c6f281fffbc150bd93 changed the way status
read buffers are handled but forgot to adjust the mempool to the new
size. Add the call to resize the mempool after the exchange config
data. Also use the define instead of the hard coded number in the fsf
callback for consistency.

[SCSI] zfcp: Do not wait for SBALs on stopped queue
Trying to read the FC host statistics on an offline adapter results in
a 5 seconds wait. Reading the statistics tries to issue an exchange
port data request which first waits up to 5 seconds for an entry in
the request queue.
Change the strategy for getting a free SBAL to exit when the queue is
stopped. Reading the statistics will then fail without the wait.

[SCSI] zfcp: Fix tracing of requests with error status
When a FSF request is returned with an error it should be reported
through blktrace for the ziomon tools, but the latency information
should not be read. Fix this by also calling zfcp_fsf_req_trace for
the error case, but skip reading the latencies inside the function.

include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

[SCSI] zfcp: Introduce header file for qdio structs and inline functions
Move the qdio related structs and some helper functions to a new
zfcp_qdio.h header file. While doing this, rename the struct
zfcp_queue_req to zfcp_qdio_req to adhere to the naming scheme used in
zfcp. This allows a better seperation of the qdio code and inlining
the helper functions will save some function calls.

[SCSI] zfcp: Remove two FIXME comments
On a link down, the adapter reopen is not strictly necessary, but it
helps flushing pending requests as quickly as possible. Add a comment
mentioning this.
qdio returning a problem on the response queue is an unlikely event.
The recovery mentioned in the comment might resolve it, so implement
it. This also has the advantage that it creates an entry in the
recovery trace to see if and when this is occurring.

Move the code for tracking FSF requests to new file to have this code
in one place. The functions for adding and removing requests on the
I/O path are already inline. The alloc and free functions are only
called once, so it does not hurt to inline them and add them to the
same file.

[SCSI] zfcp: Set hardware timeout as requested by BSG request.
The hardware used with zfcp provides a timer for CT and ELS requests
instead of an abort capability for these commands. To correctly handle
the FC BSG timeouts, pass the timeout from the BSG requests to the
hardware.

[SCSI] zfcp: Activate fc4s attributes for zfcp in FC transport class
Enable the display of supported and active fc4s for zfcp in the FC
transport class. zfcp only supports FCP, so simply hard-code this
information. The zfcp hbaapi already has this information hardcoded,
but this would allow to switch from the coding in the zfcp hbaapi to
the common FC transport attributes in the future.

[SCSI] zfcp: Update FSF error reporting
The SCSI midlayer retries commands based on the remote port state and
the command status reported by the driver. Returning
DID_TRANSPORT_DISRUPTED is a better approach, use this for reporting
FSF errors back to the SCSI midlayer. See

There is also no need in special treatment of ABORTED commands, so
remove the ZFCP_STATUS_FSFREQ_ABORTED, the commands are then returned
with DID_TRANSPORT_DISRUPTED.
Also remove the ZFCP_STATUS_FSFREQ_RETRY: It is useless, no retry is
happening in the FSF layer and nobody checks the state of this flag.

- drop redundant data from SAN trace (local s_id that only changes
during link down, ls_code that is already part of payload, d_id in

- leverage common fsf struct for FC passthrough job data, allocate it
with dd_bsg_data for passthrough requests and unify common code for
ct and els passthrough request
- simplify callback handling in zfcp_fc

The well-known-address (WKA) port handling code is part of the FC code
in zfcp. Move everything WKA related to the zfcp_fc files and use the
common zfcp_fc prefix for structs and functions. Drop the unused key
management service while renaming the struct, no request could ever
reach this service in zfcp and it is obsolete anyway.

[SCSI] zfcp: Assign scheduled work to driver queue
The port_scan work was scheduled to the work_queue provided by the
kernel. This resulted on SMP systems to a likely situation that more
than one scan_work were processed in parallel. This is not required
and openes the possibility of race conditions between the removal of
invalid ports and the enqueue of just scanned ports. This patch
synchronizes the scan_work tasks by scheduling them to adapter local
work_queue.

[SCSI] zfcp: Warn about storage devices with broken PLOGI data
After opening a remote port zfcp checks if the WWPN returned in the
PLOGI maches the WWPN of the port that should have been opened. On a
mismatch zfcp assumes that the DID just changed, queries the FC
nameserver and tries again. If the situation persists the erp will
give up.
With this strategy, if the remote port always returns the wrong PLOGI
data, the remote port will not be opened. Introduce a warning, so that
the system administrator knows why the remote port is not being opened
and to have a pointer to investigate the problem on the storage
system.

[SCSI] zfcp: Simplify and update ct/gs and els timeout handling
The recommendation for a timeout of 2 * R_A_TOV is the same for ct/gs
and els requests, so set it in the common function used for
initializing both request types. Besides, the timer inside zfcp should
only run longer than the timeout set for the channel, so 10 seconds
more should be enough (instead of 60 seconds).

The zfcp_adapter structure was growing over time to a size of almost
one memory page. To reduce the size of the data structure and to
seperate different layers, put all qdio related data in the new
zfcp_qdio data structure.

[SCSI] zfcp: Move workqueue to adapter struct
Remove the global driver work queue and replace it with a workqueue
local to the adapter. The usage of this workqueue makes this the
correct place for the structure. In addition multiple adapters won't
block each other due to the serialization of the queued work.

[SCSI] zfcp: Only collect SCSI debug data for matching trace levels
The default trace level is to only trace failed SCSI commands. Thus it
is not necessary to collect trace data for most SCSI commands since it
will be thrown away later. Restructure the SCSI trace infrastructure
to first check the trace level in a inline function and only do the
expensive data collection for matching trace levels.

[SCSI] zfcp: Fix wka port processing
Under certain conditions it is possible that a WKA port ist not opened
within the expected timeframe of half a second. In this situation
the WKA port remains in the state OPENING preventing any succeding
request to open the port. This led to unrecoverable remote ports.
Fixing this by always setting an appropriate WKA port status before
leaving the function and removing the timeout value here since it's
not needed here because the general timeout processing would deal
with it if required.

[SCSI] zfcp: Recover from stalled outbound queue
Depending on interruptions on some storage systems, the complete
channel can stall which looks like an outbound queue stall to Linux.
When trying to acquire a free SBAL for a non-SCSI command, zfcp waits
for 5 seconds for a free slot to appear. This is the right place to
detect a queue stall: If the wait times out, we assume a stalled queue
and try to recover this.
The overall strategy should be to trigger the erp from specific
events, and not try an overall escalation from one failed port to a
full-blown queue recovery. If we manage to send a command, the status
codes for this command or a timeout will trigger the right follow-on
actions.

[SCSI] zfcp: Use unchained mode for small ct and els requests
The ELS ADISC and the GID_PN requests sent from zfcp fit into
unchained FSF requests. Change the FSF allocation logic to use
unchained requests whenever possible where everything fits in one
SBAL. This avoids acquiring more SBALs than necessary, especially
during zfcp recovery when things might be stalled.

We should not modify the port status after triggering an ERP action
for the port. It is not guaranteed which status is finally active
when the ERP action is performed. This can lead to situations which

Don't access the block layer request, get the payload length instead
from the FC job. Simplify access to the zfcp_port, only the d_id is

In rare cases, open port request might timeout, erp calls
zfcp_port_put, port gets dequeued. Now, the late returning (or
dismissed) fsf-port-open calls the fsf_port_open_handler that tries to

[SCSI] zfcp: Remove unnecessary default case and assignments
enum dma_data_direction only has the 4 values DMA_BIDIRECTIONAL,
DMA_TO_DEVICE, DMA_FROM_DEVICE and DMA_NONE. No need to have the
default case. While changing this, setup sbtype in one place to make
sparse happy.
The default value of retval is already -EIO, so remove the
additional assignment for these two cases.

[SCSI] zfcp: Block FC transport rports early on errors
Use the I/O blocking mechanism in the FC transport class to allow
faster failovers for multipathing:
- Call fc_remote_port_delete early to set the rport to BLOCKED.
- Check the rport status in queuecommand with fc_remote_portchkready
to no longer accept new I/O for this port and fail the I/O with the
appropriate scsi_cmnd result.
- Implement the terminate_rport_io handler to abort all pending I/O
requests
- Return SCSI commands with DID_TRANSPORT_DISRUPTED while erp is
running.
- When updating the remote port status, check for late changes and
update the remote ports status accordingly.

[SCSI] zfcp: replace current ERP logging with a more convenient version
The current number based id ERP logging is replaced by a string
based tag version. The benefit is an easier location of the code in
question and the removal of the lengthy array referencing the
individual messages.
The string (7 bytes) based version does not use more space since those
bytes were "used" anyway due to the alignment of the structure.
The encoding of the 7 byte string is as follows
[0-1] = filename
[2-5] = task/function
[6] = section
Due to the character of this string (fixed length) a string
termination is not required here.

[SCSI] zfcp: Send ELS ADISC from workqueue
Issue ELS ADISC requests from workqueue. This allows the link test
request to be sent when the request queue is full due to I/O load for
other remote ports. It also simplifies request queue locking,
zfcp_fsf_send_fcp_command_task is now the only function that has
interrupts disabled from the caller. This is also a prereq for the FC
passthrough support that issues ELS requests from userspace.

[SCSI] zfcp: Improve reliability of SCSI eh handlers in zfcp
When the SCSI midlayer is running error recovery, the low-level error
recovery in zfcp could be running and preventing the SCSI midlayer to
issue error recovery requests. To avoid unnecessary error recovery
escalation, wait for the zfcp erp to finish and retry if necessary.
While reworking the SCSI eh handlers, alsa cleanup the code and
simplify the interface from zfcp_scsi to the fsf layer.

[SCSI] zfcp: Remove some port flags
PORT_PHYS_CLOSING is only set and cleared, but not actually used
for status checking.
PORT_INVALID_WWPN is set when the GID_PN request does not return
a d_id for a remote port, e.g. when a remote port has been
unplugged. For this case, the d_id is zero. In the erp we can
check the d_id and use the normal escalation procedure that gives
up after three retries and remove the special case.
PORT_NO_WWPN is unused: Each port in the remote port list has a
valid wwpn. The WKA ports are now tracked outside the port
list. Remove the PORT_NO_WWPN flag, since this is no longer set
for any port.

[SCSI] zfcp: Remove unnecessary warning message
Remove a message that was emitted for a port that could not initially
be opened. This is a rare case when the port discovery hits an
initiator port and only confuses the user with an initator port logged
in the message. Remove the whole special case: The failed "open port"
request triggers required follow-up actions anyway.

[SCSI] zfcp: Add support for unchained FSF requests
Add the support to send CT and ELS requests as unchained FSF requests. This is
required for older hardware and was somehow omitted during the cleanup of the
FSF layer. The req_count and resp_count attributes are unused, so remove them
instead of adding a special case for setting them. Also add debug data and a
warning, when the ct request hits a limit.

The port flag DID_DID indicates whether we know the current id of the
port. This is always set in parallel. Since the id 0 is invalid
(because the port id 0 is invalid) we can remove the DID_DID flag:
d_id of 0 indicates an invalid d_id != 0 is a valid one.

[SCSI] zfcp: eliminate race between validation and locking
The check of having a valid pointer was performed before the
processing was secured by the lock. Between those two steps the
pointer can turn invalid. During further processing another value is
used (referenced by the pointer described above) as a function pointer
which is never verified to be valid either, resulting under some
circumstances in an invalid function call. This patch is fixing both
issues.

[SCSI] zfcp: returning an ERR_PTR where a NULL value is expected
Aborting a SCSI cmnd might requrie to send a abort_fsf_cmnd. If the
creation of this fsf_req fails an ERR_PTR is returned where a NULL
value would be expected as an error indicator. This ERR_PTR is
dereferenced as valid fsf_req in succeeding processing leading to
an error.

Fix the handling of the request list in the error path:
- Use irqsave for the lock as in the good path.
- Before removing the request, check if it is still in the list, a
call to dismiss_all might have changed the list in between.
- zfcp_qdio_send does not change the queue counters on failure,
trying revert something is wrong, so remove this.

[SCSI] zfcp: fix deadlock caused by shared work queue tasks
Each adapter reopen trigger automatically a scan_port task which
is waiting for the ERP to be finished before further processing.
Since the initial device setup enqueues adapter, port and LUN which
are individual ERP actions, this process would start after
everything is done. Unfortunately the port_reopen requires another
scheduled work to be finished which is queued after the automatic
scan_port -> deadlock !
This fix creates an own work queue for ERP based nameserver requests.

Now that we removed the long messages for the bit error threshold
data, put the data in the hba trace. This way, we get a short warning
for the threshold event from the hardware and have the data in the

Reduce the size of zfcp data structures by removing unused and
redundant members. scsi_lun is only the mangled version of the
fcp_lun. So, remove the redundant field and use the fcp_lun instead.
Since the queue lock and the pci_batch indicator are only used in the
request queue, move them from the common queue struct to the adapter
struct.

[SCSI] zfcp: attach and release SAN nameserver port on demand
Changing the zfcp behaviour from always having the nameserver port
open to an on-demand strategy. This strategy reduces the use of
limited resources like port connections. The patch provides a common
infrastructure which could be used for all WKA ports in future.
Also reduce the number of nameserver lookups by changing the zfcp
behaviour of always querying the nameserver for the corresponding
destination ID of the remote port. If the destination ID has changed
during the reopen process we will be informed and then trigger a
nameserver query on demand.

[SCSI] zfcp: remove unused references, declarations and flags
- Remove unused references and declarations, including one instance
of the FC ls_adisc struct that has been defined twice.
- Also remove the flags COMMON_OPENING, COMMON_CLOSING,
ADAPTER_REGISTERED and XPORT_OK that are only set and cleared, but
not checked anywhere.
- Remove the zfcp specific atomic_test_mask makro. Simply use
atomic_read directly instead.
- Remove the zfcp internal sg helper functions and switch the places
where it is still used to call sg_virt directly.
- With the update of the QDIO code, the QDIO data structures no
longer use the volatile type qualifier. Now we can also remove the
volatile qualifiers from the zfcp code.

[SCSI] zfcp: Automatically attach remote ports
Automatically attach the remote ports in zfcp when the adapter is set
online. This is done by querying all available ports from the FC
namesever. The scan for remote ports is also triggered by RSCNs and
can be triggered manually with the sysfs attribute 'port_rescan'.

[SCSI] zfcp: Move CFDC code to new file.
zfcp implements a device file to allow Linux guests changing the
Access Control Tables stored in the adapter. The code for the device
file has nothing to do with the other parts of the driver, so move it
to a new file and cleanup the code while doing so.

[SCSI] zfcp: receiving an unsolicted status can lead to I/O stall
Processing of an unsolicted status request can lead to a locking race
of the request_queue's queue_lock during the recreation of the
used up status read request while still in interrupt context
of the response handler.
Detaching the 'refill' of the long running status read requests from
the handler to a scheduled work is solving this issue.
In addition, each refill-run is trying to re-establish the full amount
of status read requests, which might have failed in earlier runs.

Add the infrastructure to retrieve the fabric and channel latencies
from FSF commands for each SCSI command that has been processed. For

[SCSI] Let scsi_cmnd->cmnd use request->cmd buffer
- struct scsi_cmnd had a 16 bytes command buffer of its own.
This is an unnecessary duplication and copy of request's
cmd. It is probably left overs from the time that scsi_cmnd
could function without a request attached. So clean that up.
- Once above is done, few places, apart from scsi-ml, needed
adjustments due to changing the data type of scsi_cmnd->cmnd.
- Lots of drivers still use MAX_COMMAND_SIZE. So I have left
that #define but equate it to BLK_MAX_CDB. The way I see it
and is reflected in the patch below is.
MAX_COMMAND_SIZE - means: The longest fixed-length (*) SCSI CDB
as per the SCSI standard and is not related
to the implementation.
BLK_MAX_CDB. - The allocated space at the request level
- I have audit all ISA drivers and made sure none use ->cmnd in a DMA
Operation. Same audit was done by Andi Kleen.
(*)fixed-length here means commands that their size can be determined
by their opcode and the CDB does not carry a length specifier, (unlike
the VARIABLE_LENGTH_CMD(0x7f) command). This is actually not exactly
true and the SCSI standard also defines extended commands and
vendor specific commands that can be bigger than 16 bytes. The kernel
will support these using the same infrastructure used for VARLEN CDB's.
So in effect MAX_COMMAND_SIZE means the maximum size command
scsi-ml supports without specifying a cmd_len by ULD's

[SCSI] zfcp: Print some messages only during ERP
When statistics are polled from sysfs, the statistics use the same
commands as the adapter initialization. Change the messages printed
here, so they are only printed during initialization and not for each
poll of adapter data.

When a FSF physical close returns the status boxed, this means that
another system already closed the port. For our system this is the
same status as in the good path, we have to send the normal close. So,
set the status for the boxed response to the same as for the good
status.

[SCSI] zfcp: Fix evaluation of port handles in abort handler
According to the FSF spec, word 0 (bytes 0-3) has the handle
specified with the abort command and word 1 (bytes 4-7) has the
handle for the command to be aborted. Fix the if statements

[SCSI] zfcp: Locking for req_no and req_seq_no
There is a possible race condition while generating the unique
request ids and sequence numbers. Both might be read at the
same time and have the same value. Fix this by serializing the
access through the queue lock of the adapter: First call
zfcp_fsf_req_sbal_get that acquires the lock, then read and
increment the unique ids.

[SCSI] zfcp: print S_ID and D_ID with 3 bytes
S_ID and D_ID are defined in the FCP spec as 3 byte fields.
Change the output in zfcp print statements accordingly to print
them with only 3 bytes.

Invalid locking order. Kernel hangs after trying to take two locks
which are dependend on each other. Introducing temporary variable
to free requests. Free lock after requests are copied.

[SCSI] zfcp: fix: use correct req_id in eh_abort_handler
zfcp's eh_abort_handler used the wrong request ID to
identify the request to be aborted. The bug was introduced
with commit fea9d6c7bcd8ff1d60ff74f27ba483b3820b18a3
for improved management of request IDs. The bug is
fixed with this patch.

[SCSI] zfcp: create private slab caches to guarantee proper data alignment
Create private slab caches in order to guarantee proper alignment of
data structures that get passed to hardware.
Sidenote: with this patch slab cache debugging will finally work on s390
(at least no known problems left).
Furthermore this patch does some minor cleanups:
- store ptr for transport template in struct zfcp_data

Fixed various problems in opening sequence of adapters which was previously
changed with NPIV support:
o corrected handling when exchange port data function is not supported,
otherwise adapters on z900 cannot be opened anymore
o corrected setup of timer for exchange port data if called from error
recovery
o corrected check of return code of exchange config data

[SCSI] zfcp: add additional fc_host attributes
this patch adds some fc host attributes and removes its equivalents
from the zfcp_adapter structure and zfcp specific sysfs subtree.
Furthermore it removes superfluous calls to fc_remort_port_delete when
an adapter is set offline because rports will be removed by
fc_remove_host anyway.

[SCSI] zfcp: provide support for NPIV
N_Port ID Virtualization (NPIV) allows a single FCP port to appear as
multiple, distinct ports providing separate port identification. NPIV
is supported by FC HBAs on System z9. zfcp was adapted to support this
new feature.

[SCSI] zfcp: enhancement of zfcp debug features
Debug features (DBFs) els_dbf, cmd_dbf and abt_dbf were removed and
san_dbf, hba_dbf and scsi_dbf were introduced. The erp_dbf did not
change.
The new traces improve debugging of problems with zfcp, scsi-stack,
multipath and hardware in the SAN. san_dbf traces things like ELS and
CT commands, hba_dbf saves HBA specific information of requests, and
scsi_dbf saves FCP and SCSI specific information of requests. Common
to all new DBFs is that they provide a so called structured view. This
significantly improves readability of the traces.

[SCSI] zfcp: remove union zfcp_req_data, use unit refcount for FCP commands
o union zfcp_req_data removed
o increment unit refcount when processing FCP commands
(This fixes a theoretical race: When all scsi commands of a unit
are aborted and the scsi_device is removed then the unit could be
removed before all fsf_requests of that unit are completely processed.)

[PATCH] zfcp: add rports to enable scsi_add_device to work again
This patch fixes a severe problem with 2.6.13-rc7.
Due to recent SCSI changes it is not possible to add any LUNs to the zfcp
device driver anymore. With registration of remote ports this is fixed.

[SCSI] zfcp: fix bug during adapter shutdown
Fixes a race between zfcp_fsf_req_dismiss_all and
zfcp_qdio_reqid_check. During adapter shutdown it occurred that a
request was cleaned up twice. First during its normal
completion. Second when dismiss_all was called. The fix is to
serialize access to fsf request list between zfcp_fsf_req_dismiss_all
and zfcp_qdio_reqid_check and delete a fsf request from the list if
its completion is triggered. (Additionally a rwlock was replaced by a
spinlock and fsf_req_cleanup was eliminated.)

Fixes a bug in zfcp_send_els_handler. If D_ID assignments for ports
are changing between initiation of one ELS request and its completion
the wrong port might be accessed in the completion for that ELS
request. Thus a pointer to the port has to be passed for ELS requests
to identify the port structure if required.

Linux-2.6.12-rc2
Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
